He was predeceased by: his parents, Ernie Cook and Madeline Hutson Cook; his siblings, Elsie and Raymond; and the mother of his children Sandra K Kashon.
He is survived by: his daughters, Darla Lott of Belpre, OH and Rhonda Montgomery (Jamie) of Rockville; his grandchildren, Jeremy Richards of Belpre, OH, Tara Kerns (Larry) of Parkersburg, WV and Michael Powell of Columbus, OH.; and his granddaughter-in-law Sonya Collins Cook.
