He was predeceased by: his grandparents, Richard, Helen Cramer (Marsh), Forrest Sr. and Ester Blake (Schroeder). He is survived by: his wife Angelia "Angie" (Green); his daughters, Delaney and Natalie Blake; his parents, Forrest Jr. Blake and Marsha Blake (Cramer) of Bowling Green; and his brothers, Scott Blake (Erica) of Toledo, Erik Blake (Kate) of Swanton, Randolph Blake (Abby) of Norwalk and Patrick Blake (Katie) of Sylvania. He is also survived by many nieces and nephews.
Visitation was held on Thursday, September 3rd 2026 from 4:00 PM to 7:00 PM and on Friday, September 4th 2026 from 10:00 AM to 11:00 AM. A funeral service was held on Friday, September 4th 2026 at 11:00 AM at the St. Mark's Lutheran Church (315 S College Dr, Bowling Green, OH 43402).
