He was predeceased by: his grandparents, Beverly, Wayne Puterbaugh, Annalee and Leon Lewis. He is survived by: his parents, William "Bill" Joseph Lewis and Tamey Lee Lewis (Puterbaugh); his stepmother June Lewis of Springfield; his children, Nevaeh Destiny June Lewis and Nicholas Wayne Lewis; his sister Holly Jo; his brother-in-law Eric Stephenson; his niece Kaylee Jo Stephenson; and his daughter Heidi Lynn Lewis. He is also survived by other relatives and many friends.
