He was loved and cherished by many people including: his wife Gail; his sons, Jack Jr. Lezak (Kristy), James Lezak and Jason Lezak; his siblings, Susan Gibbs, Harry Lezak, Darlene Pellrite (Joe), Terry Lezak (Debra) and Michael Lezak (Carole); and his grandchildren, Paige and Peyton.
