He was predeceased by : his parents, Frederick R. Jacoby and Helen D. Jacoby (Ward); his brother James Jacoby; and his nephew Justin Meyer. He is survived by : his children, Matthew Jacoby (Meghan) of Flowery Branch, Georgia, Haley Scott (Jacob) of Berkley, MI and Alexandra Jacoby (Dylan Mason) of Bryan; his grandchildren, Olivia and Isla; his girlfriend Deb Miller of Archbold; his siblings, Sharon Liechty (Robert), Robert Jacoby (Darlene) and Nancy Vogelsong (Rex); his sister-in-law Jodi Frank (Jeff); and his pets, Lizzie and Truly. He is also survived by many nieces and nephews.

In lieu of flowers, the family asks that memorial contributions be made to CHP Defiance Inpatient Hospice.
