He was predeceased by: his parents, Jacob J. II and Thora Belle Sladkey II (Scott); and his brothers, Albert and Larry McKinnon. He is survived by: his wife Helen Perry; his son Jacob Sladkey IV (Cindy); his grandchildren, Maryssa Duncan, Drake Duncan, Jacob Sladkey V and Leita Sladkey; his sisters, Tori Pierce (Jeff), Jacqueline Pisco (Mark) and Mary Ann Bianco; and his sisters-in-law, Sandra McKinnon and Linda McKinnon. He is also survived by many nieces, nephews, and friends.
Visitation was held on Saturday, January 17th 2026 from 9:30 AM to 11:00 AM at the Grace Church of Rootstown (4808 Tallmadge Rd, Rootstown, OH 44272). A celebration of life was held on Saturday, January 17th 2026 at 11:00 AM at the same location.
In lieu of flowers, memorial contributions can be made to Wounded Warrior Project, Tunnel to Towers Foundation, or US Department of Veterans Affairs.
