He was predeceased by: his father Paul Cook; his son Christopher Blake; and his brother Terry Cook. He is survived by: his parent Kay Cook (Turner); his children, Russell Blake of Carbon Hill, Ryan Cook (Amber) of Nelsonville, Shannon Blake (Lori) of Redtown, Michael Blake (Candace) of Labrador City, Canada and Candice Cook of Carbon Hill; his grandchildren, Mercedes, Kailey, Brody, Anna, Kasin, Cassidy, Treynor, Grace, Lane, Eli, Kayson, Liam, Brady, Christian, Matthew, Abel, Trevor and McKenzi; and his siblings, Danny Cook (Wendy) of Starr, John Cook (Summer) of St. Clairsville, Eddie Cook (Caitlyn) of Logan, Cheryl Cook of Starr and Amy Perkins (Jason) of Lancaster.
Visitation will be held on Monday, October 13th 2025 from 5:00 PM to 8:00 PM at the Souers Cardaras Funeral Home (46 Fayette St, Nelsonville, OH 45764). A funeral service will be held on Tuesday, October 14th 2025 at 11:00 AM at the same location. A burial will be held on Tuesday, October 14th 2025 at the Carbon Hill Cemetery.
The family suggests donations to the funeral home to assist with funeral expense.
