He was predeceased by: his parents, Willis Ballard and Donna Ballard (Myers); his wife Patsy Marie Ballard (Lemaster); his daughter Donna Lee Ballard; his parents-in-law, Victor and Virgie Lemaster; his close friend Bob Wheeler; and his pet Cooter.
He is survived by: his children, Robin Lynn Ballard and Frank Ballard (Mary Beth); his grandchildren, Heather Winkhart (Todd), Shon Ballard, Jason Ballard (Ashley) and Amanda Lindeman (Brett); his great grandchildren, Rylan Winkhart, Blake Ballard, Kenley Ballard, Autumn Lindeman and Bram Lindeman; and his sister Joyce Bollinger (Ron).
