He was predeceased by: his parents, Glen Forwerck and Emma Forwerck (Harter); his son Justin Forwerck; his granddaughter Destiny Sullivan; and his father-in-law Ronald Hilgefort.
He is survived by: his wife Kathy Forwerck (Hilgefort); his children, Brett Forwerck (Angie), Matt Forwerck (Jeanine), Nick Forwerck, Columbus, Dr. Jason D. Springer (Betsy), Amanda E. Springer and Heather Sutter; his sisters, Kay Randolph and Jeanne Whitaker; his sister-in-law Karen Cheeseman (Brad); his mother-in-law Betty Hilgefort; and his friends, Tom Risch of Celina and Mert Nottingham of Celina. He is also survived by 14 grandchildren; 4 great grandchildren; many nieces and nephews.
