He was predeceased by: his parents, George T. Hazlett and Frances Christian Hazlett; his brothers, Ronald Pat, Thomas, Dan and Kenny Hazlett; and his son Bobby Joe Hazlett.
He is survived by: his wife Kathleen Conley Hazlett; his children, Don (Bettina Hazlett) and Billie Jo (Steve Saunders); his grandchildren, Brandon Adkins, Heath Hazlett (Brooke), Heavin Hazlett (Oakie Haynes), Amanda Todd (Chad) and Taylor Burden (Will); and his great grandchildren, Dani Lynn, Brylee, Cayden, Sloane, Hyde, Judd and Paislee.
