He was predeceased by: his parents, Vincent and Pearl; and his sisters, Gayle Presutto and Patricia Fierro. He is survived by: his wife Betty Ann; his children, Laura Chamberlain, Steven Horak (Marcy) and Matthew Horak (Shannon); his grandchildren, Jennifer Waidmann (Matt), Jay Chamberlain (Katelyn), Spencer Chamberlain (Nicole), Christopher Horak, Claire Herm (Daniel), Anna Brawner (Seth) and Samuel Horak; his great grandchildren, Owen, Bennett, Connor, Kayleigh, Anderson, Rylee, Raegan, Topher, Luka and Caleb; and his sister-in-law Toni Mellett. He is also survived by several nieces and nephews.

Memorials may be made to the Cincinnati Zoo or Hospice of Cincinnati.
