She was predeceased by: her parents, Kenneth Blades and Myrtle Blades; her husband Thomas H. Wylie Jr.; her siblings, Judy Blades, Karl Blades, Theodore Blades, Phyllis Cox, Barbara Poling and George Knothe (Chuck); her daughter Lisa Wylie; her grandson Thomas Wylie IV; and her great grandchild Gretchen Hazel Roots.

She is survived by: her children, Paula Lantz (Doug) and Thomas Wylie (Marsha); her grandchildren, Courtney Lantz, Taylor Uehlin (Jake), Kelsey Lantz and Danielle Miller (Chad); and her great grandchildren, Mason Miller, Juniper Miller, Marshall Roots, Graham Uehlin, Pierce Uehlin and Shay Uehlin.

Hazel always said, "I want my flowers when I'm alive" So in lieu of sending flowers, the family asks that you gift them to yourself in her memory.
