He is survived by: his children, Jackson, Delaney, Emmaline and Abby; his significant other Jessica Marcum; his significant other's children, Carly, Sam and Jazmine; his parents, Greg Woltz and Shirley Woltz; his sister Jennifer Wildermuth (Jess); his niece Hannah; and his nephew Luke (Sarah).

In lieu of flowers, the family is diligently working on setting up a foundation for a scholarship fund to honor Jeff's memory for students at Colerain High School.
