He is survived by: his son Max; his parents, Cookey Evers (Dave) and Donald Thomas Jr. (Stacy); his siblings, Tom Jacobs III (Sarah), Ryan Jacobs (Jackie) and Lauren Friedhoff (Tony); and his stepsister Erika Greatorex (Josh). He is also survived by nieces, nephews, aunts, uncles, and cousins.
In lieu of flowers, the family asks that memorial contributions be made to the funeral home to assist with costs, with any remaining funds dedicated to a college scholarship for Max.
