He was predeceased by: his parents, Frank Middendorf and Rosella Middendorf. He is survived by: his wife Janice C. Middendorf; his daughters, Sherry A. Fuller (Mark) of Lawrenceburg, IN and Christine M. Cavanaugh (Patrick) of Monroe, OH.; his grandchildren, Mallory Smith (Matthew) of Columbus, OH, Morgan Graff (Austin) of Newport, KY and Conner Cavanaugh of Monroe, OH; his great grandchild Finley; and his siblings, Edward, Larry Middendorf, Carol Jean Oehler and Mary Peterson. He is also survived by his nieces, nephews, cousins, neighbors and friends.
