He was predeceased by: his wives, Geneva Cook and Lois Cook; and his brother James Cook. He is survived by: his children, Christine Galvin, John Everett Cook, Sidney Cook and Andrea Middleton; and his stepson Mark Rottweiler. He is also survived by 20 grandchildren and five great grandchildren.
In lieu of flowers family suggests contributions to Hospice of Cincinnati or the Cincinnati Opera in his memory.
