He was predeceased by: his father Thomas G. Hoffman, Sr. (Tom). He is survived by: his children, Molly, Trey and Quinn; his wife Tracie; his mother Lois Hoffman; and his siblings, Debbie Faulhaber (Mike), Greg Hoffman (Linda), Patty Hoffman and Rita Spalding. He is also survived by nieces and nephews, and their children.

Instead of flowers, Tom requested that donations be made to one of two missions close to his heart.
