He was predeceased by: his parents, Nick and Julia (Bochar); and his sister Delores Nero. He is survived by: his wife Janet (Formick); his children, Gina Pilla (James), Andrea Pathko, Joseph (Jamie Steimetz) and Nicole Call (Nick); his grandchildren, Julia, Joseph, Kalyn, Johnny, Hannah, Jessica, McKenna, Domenic, Evan, Madison, Lucia, Aiden and Capri; his great grandchildren, Elena, Weston, Andrew and Lainey; and his sister Linda Powell (John).
In lieu of flowers, the family requests donations be made in Jack's name to Hospice of the Western Reserve, The Alzheimer's Association or the Cleveland Animal Protective League.
