He was predeceased by: his parents, Maximino Diaz and Maria Elena Diaz (Hernandez); and his brothers, Victor, Tom, Maximino and Pedro Diaz.

He is survived by: his children, Luis Rafael Diaz, Jr. (Ann), Anthony Diaz, Shabellie Mary Diaz and Johnny Boy Diaz; and his grandchildren, Kai Anthony and Aria Mildred Diaz. He is also survived by 8 brothers and sisters.

The family requests contributions be made in memory of Luis to the Cleveland Animal Protection League.
