He is survived by: his daughter Teresa Rothgeb (Mickey); his grandchildren, Michael Cook (Kelly), Tiffany, Julia, Briar and Blaise; his great grandchildren, Waylon, Scarlet, Lily and Otto; his siblings, Pauline Ray, Elma Goheen and Walt Cook (Rosetta); his sister-in-law Brenda Cook; and his close friends, Jimmy and Jennifer Blackburn. He is also survived by several nieces and nephews.
