She is survived by: her husband James Kazlausky; her sisters, Chris Gall and Kathy Goodall; her brother-in-law Edward; her niece Jennifer (Doug); her brother-in-law Bryan; her nephews, Jack and Luke; her extended family, William Kazlausky Jr. (Sara), Cathy Finigan and Robert Kazlausky (Carla); her nieces, Amanda, Holly, Renee and Megan; her sister-in-law Joanie Pettibone (Scott); her nephews, Andy and Alex; her sister-in-law Marie Johnston (Jeff); her niece Natalie; her nephew Nick; her sister-in-law Rose O'Brien; her nieces, Kelly and Katy; her sister-in-law Janet Kazlausky; her nieces, Emma and Scarlett; Jeffrey.

In lieu of flowers, Annette requested memorial contributions be made to Pelotonia or the Mount Carmel Foundation.
