She is survived by: her husband Joseph Blake Sr.; her children, Joseph II (Kathryn), Lisa, Michael (Michelle) and Michele Blake; her grandchildren, Abigail, Isabel, Noah, Taylor, Savannah, Rylee, Victoria, Sophia, Lauren (Lou) and Skyler; her siblings, Kelley Adams and Larry McElroy; and her nephew Samuel Adams.
In lieu of flowers, the family requests that donations be made to Nationwide Children's Hospital in memory of Debra Blake.
