He was predeceased by: his father Bernard Gundy; his wife Janice Van Gundy; his parents-in-law, Richard and Betty Hegenberger; his granddaughter Shelley Salazar; and his close friend Jane Cashdollar.

He is survived by: his children, William Van Gundy (Cheryl) and Rebecca Stanley (Scot); his grandchildren, Katrina Brierley, Samantha Stanley and Whitney Van Gundy; and his great grandchildren, Eli, Elaina, Nox and Avanna.

In lieu of flowers Jack wished for donations to South High School Alumni Assn. or Dayton Shriners Children's Hospital.
