He is survived by: his parents, Paul R. Clark and Florence Ellen Clark (Patton); his sister Kimberly Siefke (Philip); his daughter Cassady Johnson (Jordan); his grandsons, Hunter, Caleb and Levi Johnson; his nieces and nephews, Amanda Collins (John), Nicholas West (Ashley), Taylor Siefke (Bryan Pinero) and Logan Siefke; and his great-nephews, Bentley Collins, Loryan Pinero-Siefke, Porter West and Nolan West. He is also survived by many aunts, uncles, cousins, extended family and friends.

“I was so sadden to hear my cousin passed away. We were astranged in our adult lives, but as kids, I have great memories of my cool cousin Matt. Out of all the cousins, we were the closest in age. As kids I remember going to visit and having so much fun, without using social media. And how nice Uncle Paul and Aunt Ellen & Kim were to us.
