He was predeceased by: his daughter Jessica Forman; and his parents, Louis Forman and Jean Forman. He is survived by: his wife Marcey Goulder Forman; his children, Jackie Forman (David Hollander) and Scott Forman; his granddaughters, Sophia and Evelyn; his brother Donald Forman; and his brothers-in-law, Eric Goulder (Barb McClatchie) and Gerald Goulder (Debra). He is also survived by many nieces, nephews, cousins and friends.
In lieu of flowers, contributions in Rick's memory may be made to The Forman Pay It Forward Fund at Congregation Beth Tikvah or The Jessica Forman Brian Carnahan Walkabout Scholarship at Linworth Experiential Program at Worthington School District.
