He was predeceased by: his mother Geraldine L. Forrest. He is survived by: his father James D. Forrest of Lancaster; his siblings, Dale Forrest (Penny) of Lancaster and Jill Forrest (Michael Stafford) of Colorado Springs, CO; and his children, Ivy Riley (Todd) of Powell and Andrew Forrest of Columbus. He is also survived by 4 grandchildren.
