He is survived by: his son Tom (Alice); his grandchildren, Destini and Tommi; his daughter Tammy; his grandchildren, Cody (Jen) and Rylie (Duwayne); his great grandchild Ramiah (Jesse Akers); his granddaughter Olivia; his great grandchildren, Jeremiah, Zachariah and Jaziah; and his siblings, Rick (Lynn), Jeff (Vicky) and Colleen Hazelbaker (Jon).
