We bid farewell to Tammy Kosiba of Conneaut, Ohio, whose journey of life gracefully concluded on September 8, 2025 at the age of 63. Tammy touched countless lives with warmth and kindness, leaving behind cherished memories that will forever echo in hearts. She was predeceased by: her parents, Thomas Sr. Taylor and Joan Taylor; her daughter Autumn Louise Falke; and her sister-in-law Theresa Janke. She is survived by: her husband John Kosiba; her daughter Sarah Terry (Matt); her grandchildren, Alice and Jack Terry; her siblings, Timmy, Taylor, Terry Taylor and Kathi Swann (Jim); her mother-in-law Rosemarie; her brother-in-law Stan Kosiba; her close friend Ann Turner; her brothers, Golden Dawn and John; and her caregiver Erick.

Contributions may be made in Tammy's memory to Hospice of the Western Reserve or to South Ridge Baptist Church.
