He was predeceased by: his parents, Duane "Skunky" Hawthorne and Margaret Hawthorne; his grandparents, Ida and Asa Gephart; his great-grandparent Onida Hawthorne; his sister Pam Asire; and his sister-in-law Phyliss Phelps.
He is survived by: his children, Amber Bates (Robert) of Coshocton and Richard Hawthorne (Michelle) of Coshocton; his grandchildren, Brittleigh and Blake Bates; his brother Rick Phelps (Betti); his brother-in-law Jim Asire; and his close friends, Kenny Gross (Kim) and Dale Showman. He is also survived by several nieces and nephews.
