He was predeceased by: his son Cody Hornyak; and his brother Scott Hornyak. He is survived by: his parents, Carol A. Hornyak (Paulsen) and Joseph G. Hornyak Jr.; his daughter Charlette Hornyak; and his siblings, Cyndi Hornyak and Tim Hornyak (Mindy). He is also survived by many nieces and nephews.
