She was predeceased by: her son David Ford Perkins. She is survived by: her husband Richard "Rick" Bernard; her daughter Jennifer Elly Perkins (Tim Carter) of Beavercreek, OH; her daughter-in-law Alicia Perkins of Clearwater, FL; her granddaughter Kendyl Perkins of Clearwater, FL; her great grandchild Mila Perkins of Clearwater, FL; and her aunt Shirley Young.
In lieu of flowers, Claudia's family requests contributions be made to Ohio's Hospice of Dayton or the Clearcreek Township EMS.
