She was predeceased by: her parents, Fred McIntosh and Morine McIntosh (Dunn); her husband Robert Duncan; and her siblings, James, William McIntosh and Alice Shafer.
She is survived by: her daughter Laura Hoff (Dan); her grandchildren, Ashley (Scott) and Mark Guinn; her siblings, Cathey Collins (Hershel), Mary McIntosh and Tim McIntosh; and her siblings-in-law, Don Shafer and Helen Duncan. She is also survived by her extended family & many good friends.
In lieu of flowers, contributions should be made in memory of Phyllis to The Hospice of Butler & Warren Counties.
