He was predeceased by: his mother Mary Elizabeth "Bizzie" Mahaffey (Gallagher); and his siblings, Herman Mahaffey and Martha LeMonte. He is survived by: his father Charles Matthew; his sons, Richard Mahaffey (Michelle), Kurt Mahaffey (Kim) and Burt Mahaffey; his grandchildren, Nicole, Logan Mahaffey and Taylor Page (Vince); and his sisters, Rose McClain (Ken) and Evelyn Tomer. He is also survived by numerous nieces and nephews.
In lieu of flowers, charitable contributions may be made to the American Cancer Society and Ohio's Hospice Foundation.
