She was predeceased by: her parents, Thomas R. Polen and Cora Marie Austin Polen. She is survived by: her brothers, Richard Polen (Barbara), Thomas Polen (Marjorie) and Daniel Polen; her siblings' children, Diane Jackson (Steve), Heather Amato (Lawrence), Anthony Polen (Michelle), Richard Polen (Kimberly), Thomas and David Polen; and her great-nieces, Emily and Lauren Amato. She is also survived by numerous other nieces and nephews.

In lieu of flowers, memorial contributions may be made to St. Mary Education Endowment Fund.
