He is survived by: his children, Ellen Phillips, Dean (Denise) and Jeff (Marilyn); and his grandchildren, Dean Lee and Misti.
In lieu of flowers, family requests memorial contributions be made to the American Heart Association or Buckeye Hospice in Mel's memory.
