He was predeceased by: his parents, Marion Evans and Helen Evans (Carter). He is survived by: his wife Donna Lynn Evans; his daughters, Lynn Luckoski (Tom) and Jennifer Rose (Tony); and his grandchildren, Jordan, John Luckoski (Lilly), Addison and Aiden Rose.

In place of flowers, the family asks that donations be made to Ohio's Hospice of Dayton.
