He is survived by: his parents, Don Sly and Grace Sly; his former wife Nancy Hardy; his grandsons, Phillip, Ward Thomas and Logan; his wife Linda Bishop Sly; his sister Susan Sly Burke; his children, Rachel Putnam (Ralph Rupp), Ben Sly (Jamie), Trevor Sly (Kayla), Carli Decker (Eli), Phillip Gamboe, Ward Gamboe (Aundrea) and Logan Gamboe (Alyssa); his grandson Zachery Putnam; and his children, Braylon, Brogan, Bexsly Sly, Brianna Feathers, Landyn, Lyneah Sly, Daniel, Charlotte Decker, Cailynn, Ariya, Demi Jo Gamboe, Benjamin, Alaina, Ramsey Gamboe and Koen Gamboe. He is also survived by nieces and nephews.
