He was predeceased by: his parents, Paul Wells and Etta Jackson (Perkins); and his sister Karen Davidson. He is survived by: his children, Jen Chortie and Jackie Schroeder (Nick); his stepdaughters, Renee Ickes and Sarah Keiser (Justin); his grandchildren, Jacey Chortie, Jake Chortie (Sydney), Lexi Bahn and Brady Bahn; his stepgrandchildren, Izzabell Ickes and William Ickes; and his siblings, Diane Wickman, Pamela Howard, Randy Wells (Tammy), Ronnie Wells (Patty) and Jodie Kinder (Jim). He is also survived by seven great-grandchildren.

Memorial donations may be made in Dave's name to the American Cancer Society.
