He was predeceased by: his parents, Roland Cook and LaDonna Cook (Myers); and his grandparents, Floyd, Iva Myers, Howard and Mary Carolyn Cook.
He is survived by: his children, Joshua Cook (Brittnay Durant) and Megan Kimble (Donald) of Fostoria; his siblings, Kaland Cook (Kathleen) of Pleasant Hill, Kathy Babb of Fostoria and Karen Cook (Jerry Boyer) of Perrysburg; and his grandchildren, Brayden, Hunter, Gannon, Serenity, Kenzi, Paisleigh and Shyloh. He is also survived by 7 nieces & nephews 26 great nieces & nephews 31 great great nieces and nephews.
