Though Phyllis Jean Cook may have slipped beyond the veil of mortality, her essence lingers like a faint fragrance in the air, a lingering reminder of the beauty and complexity of the human experience.
She was predeceased by: her parents, Nelson Cook and Olive Cook of Bourbon, IN.; and her sister Delores (Dee). She is survived by: her husband Richard Smith; her sons, Brian and Brad; her grandchildren, Eric and Melissa; and her great grandchild Ariya. She is also survived by her two grandchildren and precious great-granddaughter.
