As we bid Phyllis Jean Cook farewell, let us not mourn the absence of her physical form, but rather celebrate the essence of her spirit that lingers in the spaces between moments. For Phyllis Jean Cook is not truly gone, but merely transformed, her energy intertwined with the fabric of the universe itself, forever dancing among the stars.

She was predeceased by : her parents, Nelson Cook and Olive Cook of Bourbon, IN.; and her sister Delores (Dee). She is survived by : her husband Richard Smith; her sons, Brian and Brad; her grandchildren, Eric and Melissa; and her great grandchild Ariya. She is also survived by her two grandchildren and precious great-granddaughter.

A celebration of life was held on Thursday, April 25th 2024 at 11:00 AM at the Coldren-Crates Funeral Home (205 W Sandusky St, Findlay, OH 45840). Visitation was held on Thursday, April 25th 2024 from 12:00 PM to 1:00 PM at the same location.
