Cook II, Janna Lyn Sutherland and Joy Birkheimer; his girlfriend Deborah Meloy; his siblings, Bill Cook, Cindy Huffman (Dale), Jeanette McCugh (Pat) and Donna Deaton; his grandchildren, Brandi, Kahla, Ian Sutherland, Bryce Cook, Breanna Mileto, Courtney and John James Shreffler; and his great grandchildren, Aubree, Nathen Francis and Oliver Shreffler.
