He was predeceased by : his parents, John Cook and Margie Cook (Cochran); his great grandchild Casey; his sister Nancy Notestine; and his brothers-in-law, Wilbert Quednau and Gene Notestine.

He is survived by : his former life partner Jeanine Risner; his wife JoAnn Szakovits of Fremont; his children, Jonny Ray Conley, Matthew Mittelhauser (Tammy) and Andrea Goble (Gary); his grandchildren, Kelly Conley, Kevin Conley, Tiffany Ross (Brandon), Megan Sweeney, Geri Morgan (Matt), Lauren Sigler (Jake), Joshua Goble and Jacob Goble; his great grandchildren, Rhemi, Delaney, Kenzley, Aiden, Josie, Michael John, Anna, Emma and Ella; and his sisters, Madonna Foos (Norbert) and Sue Quednau.

Memorial contributions may be made to Fremont Speedway Hall of Fame, ProMedica Hospice or Donor's Choice.
