He is survived by: his children, James D. Lewis (Nancy) of Gallipolis, Jon D. Lewis (Teresa) of Crown City and Melanie Jill Collins of Patriot, Ohio; his children-in-law, Randy Whitley of Dayton, Ohio and Angela Lewis; his grandchildren, Courtney Cummons (Scott), Tiffany Fourspring (Charlie), Joshua T. Lewis, Nikolas C. Collins and Jonathon T. Collins; his great grandchildren, Sadie, Rush, Flint and Charlette; and his siblings, Patricia Mason of Gallipolis, Elizabeth Shirk of Springfield and David Lewis (Iva). He is also survived by several nieces and nephews.
