He is survived by: his significant other Shirley Slack (Ellis); his sons, Mark Slack (Sally Valle) of Sardinia and Shannon Slack (Shawna) of Georgetown, Ohio; his grandchildren, Zachary Slack (Olivia) of Georgetown, Austin Slack of Georgetown and Jaiden Slack of Mason, Ohio; his great grandchildren, Peyton, Izick, Izaiah, Bryce, Adalyn and Deacon Slack; and his siblings, Terry Slack (Ronda) of Sardinia, Ohio and Bonnie Allen (Terry) of Mt. He is also survived by many nieces, nephews, family and close friends.
