He was predeceased by : his parents, Ralph Fork and Lucille Fork; his brother John Fork; and his nephew Stephen Long of Gibsonburg. He is survived by : his siblings, Denny (Xiang), Kenny (Diane), Ron (Brenda) and Linda Long (Dana); his sister-in-law Brenda Fork; his goddaughters, Kerry Jones (Michael), Jennifer Geoghan, Diane Taulker (Jason) and Danielle Fork; his nephews, Michael Fork (Bethany), Kevin Fork (Becky) and Josh Fork (Sarah); and his great-nieces and great-nephews, Alyssa, Gwen, Jackie, Caroline, Caleb, Aiden, Mason, Brady, Grant, Austin and Jameson.
