He was predeceased by: his mother Alice Nina Hazelwood; his brother Roger Hazelwood; and his nephew William Jesse Fugate (Vincent Fugate). He is survived by: his father Ferndale Hazelwood; his siblings, Trina Lynn Bowen (Mark) and William Shawn Hazelwood (Tracy); his siblings' children, Sarah Hazelwood, Dylan and Tyler Hazelwood; and his fiancée Lynda Roberts.
