He was predeceased by: his parents, John Puska and Lillian Puska (Winsted); and his siblings, Mary Smith, Helen McCreadie, Ronald Puska and John Puska.

He is survived by: his wife Rebecca Puska; his children, Jamie Pryse (Daphane), Tomi Renee Puska, Melissa Renee Jones (Jerry) and Robert Manning; his brothers, Jim Puska (Gloria) and Don Puska (Mary Lou); his grandchildren, Jerry Jr., Dallas, Maddie, Xavier and Conner; and his great grandchild Cameron. He is also survived by numerous nieces, nephews, other relatives and friends.

Memorial contributions can be made to charity of donor's choice.
