He was predeceased by: his parents, Homer Forman and Glady Forman; his wife Judy Forman (Collins) of Manchester, Kentucky.; and his children, Frank D. Forman and Dezirae Sandness.
He is survived by: his son Daryle Forman, Sr. (Jacqueline); his sisters, Ruby Wooton (Don) and Della Knapp (Bob); his grandchildren, Misty Miller (Shane), Daryle Forman, Jr. (Kelly), Michael Murray (Alana) and Douglas Burke; and his great grandchildren, Alivia, Kerrie Miller, Vivianna and Amiella Burke.
