He was predeceased by: his parents, Willard L. Phillips, Sr. and Altha Tyree Phillips; his granddaughter Jazmin Phillips; his siblings, Willard Phillips, Timmy Phillips and Mona Phillips; his sister-in-law Esther Scott; and his nephew Wayne "Bucky" Scott.
He is survived by: his wife Myra Cumberland Phillips; his sons, Dale Phillips II (Stephanie) of Columbus, Richard Phillips (Christi) of Fairborn and Clifford Phillips of Hillsboro; his brother Ronnie Phillips of Springfield; and his niece Esteé Scott. He is also survived by 6 grandchildren; 5 great grandchildren.
