It is with great sadness that we announce the death of Leslie Anne Hazelbaker of Ironton, Ohio, who passed away on July 4, 2022, at the age of 81, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
She was predeceased by: her parents, William D. Siple and Ivis Siple (Rutledge); her husband Paul David Hazelbaker (P. D.); and her brother David Siple. She is survived by: her sons, Mark Hazelbaker of Ironton and Matthew Hazelbaker of Cassopolis; her daughter-in-law Kari Hazelbaker of Cassopolis; her grandchildren, Madeline Hazelbaker of Cassopolis and Nicholas Jellison of Cassopolis; her sister Theresa Berkshire (Dan); her nephew Tom; and her nieces, Susan, Melissa, Kimberly and Lisa Huff.
